Athletic Clearance Procedures and Forms

Students who want to participate in sports must complete all of the forms provided prior to participation in the practices or competition. Any part which is not completed will cause a delay in eligibility. 

CIF – Athlete’s Eligibility Information and Parent Consent 
1. A new CIF Consent Form is required for each year. The top portion must be filled out completely. 
2. The CIF Consent Form must be signed by parent/guardian and the student athlete on all spaces provided on both the front and back of the form. 

Insurance and Health History 
1. Both must be completed and signed by the parent/ guardian and the student 
2. List insurance company and policy or medical record number. 
3. A photocopy of the insurance coverage card must be accompanied with this form, which will be kept on file with the Athletic Director. 
4. If Athlete does not have any type of health coverage, school insurance can be purchased for a minimal cost. 

Pursuing Victory with Honor 
1. Must be completed and signed by both parent/ guardian and student athlete. 

Steroid Prohibition Use Form 
1. Must be completed and signed by both parent/ guardian and student athlete. 

Publicity Authorization and Release Form 
1. Must be completed and signed by both parent/ guardian and student athlete. 

Team Heal- Consent for Treatment Form 
1. Must be completed and signed by both parent/ guardian and student athlete. 

Emergency Card Form 
1. Each coach is required to have a completed emergency form for each athlete and must be carried to all practices and contest. 
2. The medical release must be signed by parent/guardian. 

Physical Examination Form 
1. Every student-athlete is required to submit an annual physical examination prior to their first tryout for a particular sport. This must be performed by a licensed California Physician and/or Surgeon. 
2. If the exam is performed by: N.P. (Nurse Practitioner) or P.A (Physician’s Assistant) it must be co-signed by the licensed Physician or Surgeon. 
3. Each complete physical exam must be: 
    Completed on the form provided or else your physical will not be accepted
    Signed and dated by the Physician or Surgeon.
    Stamped by the clinic or doctor’s office where it was performed

Concussion Form 
1. Must be completed and sign by the students athlete and their parent/guardian. 

Academic Eligibility 
1. Each athlete must be passing in at least 20 credits and have an over-all GPA of at least 2.0 in all classes taken. Eligibility checks are taken at the beginning of every semester and at the mid-term mark. 

Incoming Freshman 
1. You must present your June Report Card for the Fall sports. 
2. After attending Banning it will be your most recent 10 or 20 week Report Card
